of your electronic signature. Once submitted, this electronic order constitutes
an electronic letter of agency authorized by your electronic signature in accordance
with specific terms and conditions. DigitalVmail’s reliance on your electronic signature,
as obtained herein, was specifically sanctioned and written into law when the Uniform
Electronic Transactions Act (”UETA”) and the Electronic Signatures in Global and
National Transactions Act (”E-Sign”) were enacted in 1999 and 2000 respectively.
Both laws specifically preempt all state laws that recognize only paper records
or handwritten signatures.

No credit check or long-term contract is required. You will simply be billed a low
monthly service fee of $14.95 that will automatically be included on your phone
bill. The monthly recurring charges for DigitalVmail will appear on the OAN/ESBI
bill page billed on behalf of DigitalVmail.
As a telephone company subscriber, you are not legally responsible for DigitalVmail
charges incurred by minors or vulnerable adults without your consent. Neither a
long distance company nor your local exchange company may disconnect your local
telephone service nor long distance service because you refuse to pay a DigitalVmail
charge by DigitalVmail, except that nonpayment of certain regulated telecommunications
charges may result in disconnection of service in Alabama, Florida, Georgia, Kentucky,
Louisiana, South Carolina and Tennessee.
DigitalVmail is not affiliated with your local telephone provider.
There is no long term commitment and you may cancel at any time, with no cost to
